Parties to the Contract means the Seller and the Buyer as named in the main body of
the Purchase Order.

Non-Refundable Deposit. Five hundred United States dollars ($500.00)USD of the
Reservation Fee must be paid by Customer to Vive Plastic Surgery within five (5) days
of entering into this Agreement. The parties acknowledge that the nature of Vive Plastic.

Surgery’s business requires long term planning and that acceptance of a reservation
booking may result in the loss of other potential bookings for the desired date of service.
Accordingly, this five hundred dollar deposit is non-refundable without regard to the
reason for or timeliness of cancellation. Moving the Reservation Date is subject to a
rescheduling fee.

This Agreement is not effective, and Vive Plastic Surgery shall have
no obligation to hold open or available any Reservation Date, until the five hundred
dollar deposit ($500)USD deposit is actually received by Vive Plastic Surgery (including
bank processing and actual receipt of funds). Draft for five hundred dollars ($500)USD
Non-refundable deposit received. THE PARTIES ACKNOWLEDGE THAT, AS OF THE
ADDITIONAL DEPOSIT DATE, AND AT ALL TIMES THEREAFTER, THE DEPOSIT IS
NON-REFUNDABLE.
